completed and signed by the applicant.

passport-sized photograph,
affixed to the above completed application form, the photograph MUST be similar
in appearance to the photograph in the passport.
Passport should
be in order and valid for more than
six months.
Letter from your employer confirming employment, that says
you are going to visist Indonesia for the specified number
of days and that you will return back to resume duties in
the same company.
The most recent bank statement showing you have more than
£1000.
A printed copy of the flight details obtained from
your travel agency.
Cheque payable to Gulfvisa. The fees are as follows in Pound
